A hidradenite supurativa, ou acne inversa, é uma doença inflamatória crônica da pele que afeta o folículo piloso. Ela ocorre principalmente em áreas de fricção ou úmidas, como axilas, região genital e anal. As lesões podem ser superficiais ou profundas.

A hidradenite supurativa não é causada por falta de higiene, mas por fatores genéticos, hormonais e imunológicos. A doença pode ser desencadeada por alterações hormonais, como na puberdade ou gravidez, e agravada por fatores como cigarro e excesso de peso. Embora infecciosa, não é contagiosa, e pode causar complicações como dor crônica e inchaço devido à cicatrização das crises.

O diagnóstico da hidradenite supurativa é clínico, baseado na avaliação das lesões, localização e evolução. Pode ser difícil nas fases iniciais, mas quanto mais cedo for feito, mais eficaz será o tratamento, com menos impacto na vida do paciente.

A hidradenite supurativa possui três estágios: no estágio 1, há abscessos sem fístulas ou cicatrizes. No estágio 2, há abscessos recorrentes, fístulas e cicatrização. No estágio 3, a doença é grave, com múltiplas fístulas interconectadas e abscessos em áreas anatômicas completas.

O tratamento da hidradenite supurativa envolve quatro pilares: não medicamentoso (higienização, controle de doenças e estilo de vida), medicamentoso tópico (antibióticos como clindamicina), sistêmico (antibióticos por longo período e biológicos como Adalimumab), e cirúrgico (drenagem, desbridamento e correção de fístulas e cicatrizes). Com o tratamento adequado, é possível controlar a doença e melhorar os sintomas.
